Midlands and Lancashire Commissioning Support Unit( CSU) The Medicines Safety Assurance Tool is an innovative way to record, and act upon, safety alerts associated with medicines and medical devices. It can be tailored to local needs, informing CCG members of safety alerts and levels of risk so that they can make appropriate clinical decisions, thereby minimising patient harm.
Midlands and Lancashire Commissioning Support Unit U-Assure allows staff to capture their work, their productivity, the cost to the organization and ability to report to organisations we support within the NHS about where their resources have gone. U-Assure is an automated corporate governance model, currently supporting the information governance asset register, data mapping and information risk assessment requirements.
National Institute for Health Research( NIHR) Collaboration for Leadership in Applied Health Research and Care( CLAHRC) West Midlands Two interventions to encourage weight reduction in adults and children were assessed using randomised control trials by CLAHRC West Midlands, and were found to be no more effective than other available solutions. CLAHRC has promoted the research, leading to one service being withdrawn and decommissioned in the West Midlands, and the other prevented from being rolled out at scale.
Newcastle-under-Lyme South Locality Group There is increasing understanding that the early recognition and management of frailty in community settings can help to improve outcomes for older people. The introduction of elderly care facilitators project was designed to offer improved care and support to older people by assessing their needs, raising their awareness of existing local services and helping them to access services most appropriate to their needs.

Prime GP The Prime75 + project was set up to tackle head-on the issues related to loneliness. Just over twelve months on, this personal and proactive approach is seeing phenomenal results – from a 26 % decrease in appointments from high demand patients to significant cost reductions in unplanned admissions, and increased levels of patient( and practitioner) satisfaction.
Sandwell and West Birmingham Hospitals NHS Trust The innovative nature of our unique apprenticeship project was recognised in December 2015 by a visit to the accommodation block at Hallam Close, Sandwell Hospital by HRH Prince William, Duke of Cambridge, during a visit to the West Midlands. To date the project has gained three local and regional recognition awards for partnership working and providing equality of opportunity.
Sandwell and West Birmingham Hospitals NHS Trust Flo mobile phone texting service was implemented in our CCG to improve adherence to blood pressure( BP) treatment by engaging patients. In 100 patients, medications were adjusted in 27 % and 46 % were discharged with good BP control preventing 107 out-patient appointments thus saving £ 11,663 against a total cost of £ 2,329( total savings £ 9,334).
Sandwell and West Birmingham Hospitals NHS Trust We devised a new stand-alone insulin prescription chart resulting in 23 fewer insulin errors, a new diabetic ketoacidosis( DKA) protocol and app with significantly improved DKA management. The project focused on education of junior doctors and nursing staff, introduced a daily hypoglycemia email alert helping target specific problem wards and a new“ six steps to insulin safety” mandatory online module, which has seen over 73 staff competent within four months.
Staffordshire Exercise Partnership Signposting by a clinician to the best internet sites to improve exercise will create wide health benefits and reduce the strain on the NHS. StEPpep cards are positioned on the clinician’ s desk and handed out to the patient at the opportune moment,“ making every contact count” more.
Stoke-on-Trent Clinical Commissioning Group( CCG) Improving the safety of over-the-counter medication sales at non-pharmacy retail stores, this programme was led, designed, delivered and reported by year 4 medical student Phill Molloy and the twelve medical students in the mystery shopper team, recruited and trained by Phill, and with underpinning support from a local lead GP, pharmacist and lay group.

University Hospitals Birmingham NHS Foundation Trust SMS has delivered proven benefits to the NHS and the patients we care for.
