Death Grant Expression of Wish form
When reviewing pension arrangements, it is a good time to review and amend the expression of wish form as circumstances change, in order to keep up to date any change of address details for you or your beneficiaries. Completed forms are held with Devon Pension Services. If you joined the pension scheme but did not complete the expression of wish form, you can obtain a blank form from HR. The completed form should be returned to Devon Pension Services, address details are on the form.
