It is not sufficient to outline the circumstances that occurred – the link between your circumstances and your performance must be clearly stated. Supporting documentary evidence which relates clearly to the period during which your performance was adversely affected is required e.g. letters from solicitors or local councils; hospital appointment cards; medical certificates; statements from health / social care professionals are examples of acceptable evidence.
