In compliance with New York State Law, new health forms / updated health forms are required:
For all new students
• Prior to the school year in which the child reaches 5
• Dental examinations are required in the kindergarten year
Completed examination forms must be submitted prior to the first day of school, as this is part of Lamplighters Yeshivah’ s reporting requirement to the State of New York Board of Education.
Students without a health form on file will not be allowed in to school.
IMMUNIZATION REQUIREMENTS
The New York Department of Health requires each child to be current with the following immunizations as listed below:
• Measles – after 15 months and prior to entering school
• DPT Booster – upon entrance to school, booster after 4 years
• Oral Polio – 4 doses before entrance to school, booster after 4 years
• Rubella – after 1 year
• Mumps – after 1 year
• TB – Current test on file for all students( or a physician’ s letter stating the student is not at risk)
• Lead Test – Current test on file for all students
Exact dates of immunization must be indicated on the medical form completed by the physician. Please check the form before submitting it to the school to be sure that all information is properly completed.
By law, parents of NY students may file a religious exemption to immunization requirements. If this exemption applies to your family, it must be held on file in lieu of immunization records.
MEAL PROGRAM
It is our goal to provide a healthy environment for all our students. To that end, we offer 2 healthy snacks and 1 hot lunch daily( kosher, cholov yisroel, pas yisroel and bishul yisroel).
We will try our best to accommodate the special needs and allergies of your child. In certain instances, it may be deemed best for a child to bring lunch and snacks from home in order to insure his or her safety. Kashrus standards of food brought from outside( for birthdays and individual snacks / lunches) must be followed in order to maintain certain standards for everyone. Please make sure all outside food is pas Yisroel, bishul Yisroel and cholov Yisroel. If a child has food that does not follow these guidelines, we will hold the food and notify you by phone. YOUR CHILD’ S EXEMPTION FROM THE MEAL PLAN MUST BE PRE-ARRANGED. Students MAY NOT bring snacks and foods without this approval, as it causes a very difficult disruption among students and distracts them from learning time. In addition, bringing outside food can potentially be dangerous( even life threatening for some) for the children who have allergies in each class.
EMERGENCY PREPAREDNESS
Snow Days The safety of our students and staff is our utmost priority at Lamplighters. In the event of a snow day or inclement weather, we generally follow what local Jewish and public schools announce. We will let you know our decision via text blasts, Facebook and emails.
