More than one episode of deliberate self-harm
Harmful use of alcohol in the previous 2 years, or any history of alcohol dependence
Harmful use of drugs in the previous 2 years, or drug dependence in the previous 3 years
Bulimia in the previous 12 months, or any history of anorexia
Abdominal and digestive disorders, including diet
Current abdominal, digestive, or liver disease including any untreated hernia
Any history of Crohn’ s disease or Ulcerative colitis
Irritable bowel syndrome requiring medication or medical followup within the last 2 years
Reflux / indigestion requiring daily medication
Splenectomy
Eye conditions
Eye disease, for example glaucoma, keratoconus, retinitis pigmentosa
Double vision
Corneal grafts or recurrent corneal ulcers
Cataract or cataract surgery
Blind in one or both eyes
Urogenital conditions
Confirmed history of kidney, bladder, or ureteric stones
Polycystic kidney disease
Kidney transplant
Endocrine conditions
Diabetes mellitus
Adrenal gland disorders
Pituitary gland disorders
