Psychiatric conditions
o More than one episode of deliberate self-harm o Harmful use of alcohol in the previous 2 years, or any history of alcohol dependence o Harmful use of drugs in the previous 2 years, or drug dependence in the previous 3 years o Bulimia in the previous 12 months, or any history of anorexia
Abdominal and digestive conditions, including diet o Current abdominal, digestive, or liver disease including any untreated hernia o Any history of Crohn’ s disease or Ulcerative colitis o Irritable bowel syndrome requiring medication or medical follow-up within the last 2 years o Reflux / indigestion requiring daily medication o Splenectomy
Eye conditions o Eye disease, for example glaucoma, keratoconus, retinitis pigmentosa o Double vision o Corneal grafts or recurrent corneal ulcers o Cataract or cataract surgery o Blind in one or both eyes
Urogenital conditions o Confirmed history of kidney, bladder, or ureteric stones o Polycystic kidney disease o Kidney transplant
Endocrine conditions
o Diabetes mellitus o Adrenal gland disorders
o Pituitary gland disorders 5
