with the members of our church family. This is general information and we recommend you consult
with your Family Health Provider for more specific
information regarding your individual healthcare
needs. Enclosed is a list of local Dental facilities that
may provide low to no-cost dental care to those that
qualify along with other helpful links.

$108.00 includes visitation fee and 1 simple extraction (more
extractions are an additional cost). Payment plans are available
to eligible patients for extensive dental treatment.

4863 Pulaski Highway – Appointment only. Check-ups, cleanings, x-rays, fluoride treatments, and emergency dental care for
all eligible patients. Eligibility requirements are age 20 or
younger and eligible for Medicaid or Maryland Children’s
Health Program, age 60 and over who meet poverty guidelines,
pregnant or nursing mothers. Ages 22-59 are eligible for emergency services and $80 cleaning only.
