Christopher Lynch Director of Operations Lakeville Ambulette Transportation LLC Parker Jewish Institute for Health and Rehabilitation Bringing Health and Wellness to the Community 1 How would you describe your Action Learning Project and the difference you hope it will make to advance your organization’s mission (or the field) and benefit the individuals you serve? Lakeville Ambulette Transportation LLC in cooperation with Parker Jewish Institute for Healthcare and Rehabilitation, Agewell, and PSE&G will coordinate a Health and Wellness Fair at the headquarters for Lakeville Ambulette. We will provide through our vendors health screening (diabetes and blood pressure checks) along with education on health related and financial related issues. The health fair is being conducted for the community of Hempstead, New York. 2 How have the relationships you’ve developed as part of your project (with mentors, partners, stakeholders, etc.) influenced this direction and you as a leader? The team consists of representatives from Lakeville Ambulette Transportation, Parker Jewish Institute and Agewell New York. They have proved to be both knowledgeable and enthusiastic in developing the plan to create this new health fair for the community of Hempstead. The direction of the event has been most influenced by the team assembled. Senior management’s role in influencing the project has come in setting the parameters in which we are to ope
