Send your completed medical questionnaire either via email to the safe email address Jennifer will provide or mail prior to testing . Or , you can bring the form with you the day of the test . |

3 . Qualitative
Capacity for testing : one hood . Do not have masks available for purchase . Participant must bring his / her own mask ; more than one size is recommended .
1 . An OSHA questionnaire is filled out by the employee and delivered to the LVHN Occupational Medicine clinic . 2 . Completed forms are reviewed by the medical team and signed off for testing to occur . 3 . Fit Test is performed .

Will travel to an office for groups of 10 or more for an additonal fee . You must bring a KN95 mask with you . The clinic does not provide them .

Important : before you leave your fit test , please be sure to request the OSHA documentation to verify that you completed the fit test .
1 . Each person completes the respiratory questionnaire that is then reviewed by a medical provider on staff . 2 . The fit test is performed . 3 . Participants are given a certificate that states they are medically qualified to wear a mask and a statement is made that they have been fitted for the mask that is used $ 60 per test . Can be paid for with debit or credit card . Practice can be billed if they would like to arrange that .
Do not have masks available for purchase . Capacity for testing : two hoods available in Washington and one in Waynesburg .
