To Make an Appointment

Appointments are required . Before testing , your office must create an account with Sara . Once your office ' s account is created , staff can move forward to schedule appointments . It is possible for individuals to self-pay ; please discuss this option with Sara .

Appointments are not required but recommended . Call the office directly to schedule an appointment . If a group from your dental office will be taking fit tests at this location , please set up an account with Pivot . Account enrollment for dental offices includes : contact person from practice who will receive all test results and a billing contact person . If you are an individual who is not testing as part of a group , you can pay via credit card at the office .

1 . Sara will create an account for you / your office . 2 . Remotely complete OSHA respirator questionnaire for medical clearance . 3 . Once medical clearance is confirmed , fit test will be performed . Participants must bring their mask ( s ). 4 . If applicable , fit test documentation is sent to the designated contact at office .

Review of questionnaire : $ 23.60 Fit test : $ 35 Total = $ 58.60 Physical , if needed : $ 46

1 . Complete and return OSHA medical evaluation . 2 . Completed evaluation is reviewed by the medical team and approved for testing to occur . 3 . Fit test is performed . Participants must provide their own mask .

OSHA questionnaire : $ 30 Fit test : $ 43 Total = $ 73 PFT , if needed : $ 35 Physical , if needed : $ 75

Capacity : 1 participant at a time . They also offer onsite testing for groups of 10 or more ; $ 87 per hour on appointments are encouraged .

Participant must bring his / her own mask . Must be Hours : 8:30 a . m . to 3:30 p . m . Monday-Friday . clean shaven to take fit test .

Important : before you leave your fit test , please be sure to request the OSHA documentation to verify that you completed the fit test .

Appointments are required . Call clinic at ( 610 ) 326- 2300 to make an appointment . You are required to set up an account if multiple people from your office will be fit testing at this clinic . Individuals are welcome to self-pay , but also can create an account with the clinic .

Hours : 7:30 a . m . to 5 p . m . Monday- Thursday ; 7:30 a . m . to 4 p . m . Friday . You will be given an appointment time . Please wear a mask to your appointment ; you also must be clean shaven , if applicable .
