Process for Testing
1 . OSHA medical evaluation completed . 2 . Completed evaluation is reviewed by the medical team and approved for testing to occur . 3 . Fit test is performed . 4 . Final paperwork is reviewed and signed the returned .
1 . Complete and return OSHA medical evaluation . 2 . Completed evaluation is reviewed reviewed by the medical team , which will approve whether or not testing needs to occur . You would then be notified of that and schedule an appointment for fit testing to be performed . * Please write legibly on the evaluation form so the process will move forward smoothly . Note that the provider may feel that a physical needs to be performed prior to fit testing . 3 . Fit test is performed .
1 . Complete and return OSHA medical questionnaire . 2 . Questionnaire is reviewed and approved by medical staff . 3 . Qualitative fit test is performed , using bitrex and saccharin .
