Process for Testing
1 . Schedule appointment . 2 . Complete and return OSHA medical evaluation . 3 . Completed evaluation is reviewed by the medical team and approved for testing to occur . 3 . Fit test is performed .

1 . Complete and return OSHA medical evaluation form . 2 . Completed evaluation is reviewed by the medical team who will approve the testing . If there are no issues that need to be addressed from the medical evaluation , participant will be approved to have fit test . If concerns arise from medical evaluation , a physical exam will be required . 3 . Fit test is performed . Bring your own mask .
1 . Complete and return OSHA medical questionnaire . Vendor uses 3M for medical questionnaires . All staff should complete questionnaire before vendor arrives to complete fit test . 2 . Completed questionnaire is reviewed and approved for testing to occur . 3 . Fit testing and some training on proper use of masks is performed .

Additional Notes
Capacity for testing is determined by appointments . WellSpan schedules to allow the best options for the customers . Each practice location has multiple staff that are certified to perform the testing . Participant must bring his / her own mask .
Capacity for testing : one hood available . Participants can purchase masks for $ 5 per mask . Please contact Allegheny Medical to confirm the type of masks that are available for sale .
Capacity for testing : satellite locations - 1 hood ; Williamsport - 4 hoods . Participant must bring his / her own mask .
Capacity for testing : prefer to test one or two participants at a time ; can accomodate a large group . No minimum or maximum number of participants .
Important : before you leave your fit test , please be sure to request the OSHA documentation to verify that you completed the fit test .
