Hazing & Initiations
Hazing and initiations of student-athletes by other student-athletes or members of the coaching staff will not be tolerated . Illegal activity related to hazing and initiations will be reported to the police ; those involved may be subject to police action .
Emergency & Accident Procedures
• Each head coach and the Athletic Director will be First Aid and CPR-certified .
• Each practice or game must be supervised by at least two adults . One of the adults must be either the head coach or the assistant coach . At least one supervising adult must be First Aid and CPR-certified at all practices and games .
• Each athletic team will be provided with a medical kit .
• All athletes must submit a completed Athletics Medical Form prior to participating in any athletic activity . This form must be completed annually on or after July 1 st , and must be submitted as part of a complete Athletics Participation Packet .
• Athletes are not permitted to access the medical kit ; only coaches may remove items for use .
• Medical kit items are not to be used for athletes ’ uniforms , equipment , or other styling / accessory needs .
• Coaches are responsible for keeping the medical kit properly supplied .
• Coaches must check student medical information forms for student allergies and medications .
• Coaches must keep a copy of all medical information forms in the medical kit .
• Coaches must complete a student accident report within 24 hours of an accident / injury and submit the report to the Athletic Director . Direct contact to the Athletic Director must be made by the coach as soon after the accident / injury as possible .
• Coaches must make initial contact with parents at the time of injury or accident , and documentation of that notification must be presented to the Athletic Director within 24 hours .
• The Athletic Director will report the accident or injury to the Head of School .
• The Athletic Director will give a copy of the completed student accident report to the school nurse .
• In the event of serious injury , the coach or assistant should call 9-1-1 . 1 . An appointed adult must meet the ambulance and direct them to the injured athlete . 2 . If there are two coaches present , one coach should accompany the injured athlete . 3 . The head coach should do the following :
• Stay with the injured athlete until medical assistance arrives .
• Contact the athlete ’ s parents or other designated emergency contact .
• If another CCS parent is on location , ask if s / he would accompany the injured athlete to the hospital .
• Do not leave the site with the team until the injured player has departed with the ambulance .
