2026 Better Together Community Benefit Grant
Request for Proposals
Project Eligibility: Better Together Grants Projects Must:
• Be evidence-based, promising or best practices as the basis for projects.
• Apply a social determinants of health framework.
• Align with current community health need assessment priorities, and existing community health implementation plan.
• Include routine performance reporting and program evaluation.
Reporting: Grantees of the Better Together Grants agree to:
• Communicate effectively with Renown Community Benefit Staff.
• Report data on their program or project bi-annually( 2 times per year).
• Participate in community meetings held bi-annually( 2 times per year).

• Organization description and mission statement: o Briefly describe your organization, and its role in the community. Please include the mission statement of the organization.
• Program Focus: o Please pick 1 priority area from the dropdown menu. o Please pick 1 strategy from the list.
• Program Information: o Program Description:
• Provide a clear overview of the program you are proposing. This section should include:
• The purpose of the program, a detailed timeline, and priority population( s). o Lives Touched:
• Provide the amount of people anticipated to be reached through the program. o Alignment with Renown Health:
• Please provide a brief description of how the program aligns with Renown Health ' s mission, vision, and values. o Evaluation Method:
• Describe how you will assess the program’ s effectiveness. Address the following:
• The metrics and indicators you will use to measure success and the timeline for assessment.
• The evaluation tools or methods( e. g., surveys, interviews, data analysis).
• How the results will be used to improve the program or guide future initiatives. o Collaborative Partners:
• Identify the partners who will contribute to the success of your program. o Sustainability:
• Explain how the program will continue to deliver benefits beyond the grant period.
• Financial Information o Total program budget and requested grant amount.
