• Minimally invasive aortic , mitral , pulmonic and tricuspid valve repair and replacement using sternal sparing , port-access approaches
• Transcatheter aortic valve replacement ( TAVR )
• Transcatheter mitral valve repair ( TMVR ), especially for high risk surgical patients with severe mitral regurgitation
• Transcatheter valve-in-valve implantation for failed bioprosthetic valves in the aortic and mitral valves
• Acute endocarditis
• Carcinoid heart disease
• Degenerative disease or congenital anomalies
• Ischemic cardiomyopathy
• Mitral annular calcifications Treatment options :
• Medical management
• Minimally invasive procedures
• Percutaneous balloon valvuloplasty
• Percutaneous mitral balloon commissurotomy or surgical commissurotomy
• Surgical repair or replacement
• Transcatheter procedures
• Valvulotomy
• better long-term patient survival ,
• better preservation of heart function ,
• lower risk of complications , and
• usually eliminates the need for long-term use of blood thinners ( anticoagulants ).+

Valve Disorders Center
A part of the comprehensive structural heart disease program , the Valve Disorders Center at Baylor Scott & White Heart and Vascular Hospital – Fort Worth celebrated a milestone of completing its 500th transcatheter aortic valve replacement ( TAVR ) since the program ’ s inception in late October 2017 . This is a milestone few cardiac specialty centers have achieved .
With referrals coming from all over Tarrant County and the western region of Texas , the center offers care in Fort Worth , as well as evaluation , pre-op and post-op care through outreach clinics in Amarillo , Lubbock , Odessa , and Weatherford .
The center ’ s multidisciplinary team meets to review each potential patient ’ s case and determine the most appropriate treatment , starting with the least invasive approach . The team emphasizes the importance of specialized care early in the disease process , guiding treatment before valve disease becomes severe .
Therapies available include :

• Minimally invasive aortic , mitral , pulmonic and tricuspid valve repair and replacement using sternal sparing , port-access approaches

• Transcatheter aortic valve replacement ( TAVR )

• Transcatheter mitral valve repair ( TMVR ), especially for high risk surgical patients with severe mitral regurgitation

• Transcatheter valve-in-valve implantation for failed bioprosthetic valves in the aortic and mitral valves

Mitral valve program
The incidence of mitral valve disease ( MVD ) is growing as the overall population ages . Mitral valve disease describes the condition when the mitral valve doesn ’ t work properly because its leaflets don ’ t close tightly , allowing blood to flow back into the left atrium , known as mitral valve regurgitation ( also known as a leaky valve or heart murmur ). When this occurs , the heart doesn ’ t pump enough blood out of the left ventricle to supply the body with oxygen-rich blood . Collaborating with the cardiothoracic surgeons on the medical staff at Baylor Scott & White — Fort Worth , the mitral valve program ’ s specialists treat both degenerative mitral valve disease as well as functional mitral valve conditions .
For patients with mitral valve regurgitation , MitraClip © is commonly used to close the mitral valve flaps tighter . During a minimally invasive procedure using a catheter guided through a vein in the leg to the patient ’ s heart , a small clip on the end of the catheter is attached to the mitral valve to help it close more completely . This helps to restore normal blood flow through the heart .
Other patients may present with :

• Acute endocarditis

• Carcinoid heart disease

• Degenerative disease or congenital anomalies

• Ischemic cardiomyopathy

• Mitral annular calcifications Treatment options :

• Medical management

• Minimally invasive procedures

• Percutaneous balloon valvuloplasty

• Percutaneous mitral balloon commissurotomy or surgical commissurotomy

• Surgical repair or replacement

• Transcatheter procedures

• Valvulotomy

Total transcatheter mitral valve repair
( since beginning of program in FY18 )
Although mitral valve repair is more surgically complex than replacement , it reduces the need for blood thinners and complications of prosthetic valves . Research indicates early mitral valve repair leads to better outcomes for symptoms , such as ventricular dysfunction or dilation , pulmonary hypertension and atrial fibrillation . In addition , mitral valve repair provides :

• better long-term patient survival ,

• better preservation of heart function ,

• lower risk of complications , and

• usually eliminates the need for long-term use of blood thinners ( anticoagulants ).+
