3 Urine Drug Screen Urine drug screen must be completed as directed through Complio within 30 days of registration Random drug screening may be required . A positive result in a drug screening will result in dismissal from the Nursing program .
4 Tuberculosis Screening Initial TB screening : results of QuantiFERON-TB Gold ( blood test ) or T- Spot ( blood test ) * A tuberculin skin test ( PPD ) is not sufficient for this initial screening . Annual TB screening : results of QuantiFERON- TB Gold or T-spot or tuberculin skin test ( PPD )
5 Influenza Vaccine ( based on annual flu season ) Due when available for current flu season and before October 1
6 American Heart Association BLS CPR certification ( renewed every 2 years to maintain compliance )
7 Proof of Receipt of three ( 3 ) Hepatitis B vaccines
