topside Volume 114, Issue 2 Page 10 Human Resources, continued The following are the proper procedures in submitting Disenrollment Requests to the DSO-HR for each of the above: Member Requests (Disenrollment or Retired Status): Requests must be submitted by the member in writing to the FC. The FC forwards an ANSC Form 7035, Change of Member Status, to the DSO-HR by surface mail or scanned pdf. document: Section I: Flotilla Commander: Fills in the Member Name and Member Number only. Section II: Member: Fills in Flotilla Number (053-XX-XX) and Date; Checks either the box “I desire Retired Member status” and fills in his or her Enrollment Date OR checks the box “I desire to disenroll” and fills in the reason (optional). Note: Transfers will be addressed in a future article. Checks the box “My membership card is enclosed” and encloses card. Note: Failure to return the I.D. Card will not hold up the action, but it should be requested. Signs Section III: Flotilla Commander: If the member is requesting Disenrollment, checks the box “Recommend disenrollment effective,” fills in a date, and checks the box “at Member’s Request” OR if the member is requesting Retired Status, checks the box “Member desires and is eligible for Retired Member status and checks the box either “yes” or “no.” Signs and Dates. Important Note: If the member is in AP Status, the DCDR must also sign and date. Forward the form (and the I.D. Card) to the DSO-HR. Note: If the member returns the card, it must be sent to the DSO-HR, as DIRAUX collects the I.D. Cards at each D-Train. Section IV: DSO-HR: Reviews the form and the information; returns to FC if more is needed. If good to go, Checks the box “Recommend Disenrollment” OR the box “Member desires and is eligible for Retired Status,” as appropriate. Signs, dates, and forwards to DIRAUX with a copy to the FC.
