IV. Age-Group Chair Responsibilities A. Compile and update a calendar for their Age-Group for submission to the Program Director by September 1 st of the Program Year. The Calendar should at minimum provide the monthly Age-Group Sponsorship assignments from September – May of the Program Year. Co-Sponsorship between Members is allowed as long as the balance of the months have sufficient coverage. Goal: children should have some sponsored activity each month from September to May. If there is any conflict between this rule and the Programming Procedures, the Programming Procedures shall govern. Sample Calendar attached as Exhibit B (feel free to use your own format). B. Distribute the calendar to the Members of the Age-Group as needed C. Ensure that at least one Age-Group community service project is executed within a given program year. D. Combining Age-Group activities is allowed as long as the activity is age-appropriate for the Age-Group the Chair serves. E. Combining Age-Groups for a community service project is allowed as long as the service is age-appropriate for the AgeGroup the Chair serves. F. Record attendance at each Age-Group Activity for submission to the Program Director within 30 days of the activity. Sample Sign-in sheet attached as Exhibit C (feel free to use your own format). G. Ensure that your Age-Group is supporting National, Regional and Chapter initiatives through and/or within the Age-Group activities scheduled throughout the year by encouraging Members to plan activities with the initiatives in mind. H. Collect Medical Information and Liability Release Forms (sample attached as Exhibit D, but please ensure that you use the most current version promulgated by Nationals) for EVERY child in your Age-Group (prior to participation in any Age-Group Activity). These forms should be submitted to the Program Director by her stated deadline, but no later than prior to any child’s participation in an Age-Group Activity. NOTE: THIS IS A NATIONAL REQUIREMENT - NO EXCEPTIONS! I. Collect Medical Information and Liability Release Forms for EVERY child that participates in an Age-Group Activity with our Member-children (prior to participation in any AgeGroup Activity). NOTE: NO EXCEPTIONS! J. Collect Youth Service Recognition Forms (sample attached as Exhibit E) and deliver them to the Teen Sponsor by her stated deadline. K.
