Population Served Report ( PSR )

1 . Purpose The PSR gives the statistics of demographic and income data for all clients served with HUD grant funded programs . The PSR is now located on the first tab of the CDS . The new PSR automatically pulls clients ’ demographic data from the CDS . There is no need to enter any data on the PSR , it is all done for you . All cells are locked in the PSR spreadsheet to prevent tampering with formulas . The PSR tab will be very useful in completing the PSR in ZoomGrants .

2 . Data Needed All cells on the PSR are locked . Formulas have been written into the spreadsheet to automatically calculate all totals from month to month . Numbers entered into the CDS are automatically pulled into the PSR ; therefore , data entry into the PSR is no longer needed . The newly updated forms for PY2020 must be used , older versions of the forms will not be accepted unless previous arrangements have been made with Grants Management .

Client Service Log ( CSL )

1 . Purpose

• Counts the number of services provided for the month
• The CSL tells which days of the month each ALL clients were served as well as the number of services each client received ;
• Keep a record of all services provided to all clients served during the program year so they can be reviewed when your agency is monitored .

2 . Data Needed The cells colored green are the only cells that require your data entry . The green cells are the only cells that subrecipients should fill in . Formulas have been written into the spreadsheet to automatically calculate all totals from month to month . The newly updated forms for PY2020 must be used , older versions of the forms will not be accepted unless previous arrangements have been made with Grants Management . Once the spreadsheet has been completed for monthly reporting , the Excel spreadsheet itself can be uploaded into ZoomGrants . No scanning is needed .

Performance Measurement Report ( PMR )

1 . Purpose

• The PMR tracks agencies performance towards the contracted goals set for the program year ;
• Each agency receiving a HUD grant is expected to meet or exceed its goals .
