inspection to steps, platforms, all support points such as pivots, rollers etc>,, all
suspension points such as lugs, brackets etc, stanchions, rigid handrails, hand ropes,
turntables, side-stringers, cross-members as applicable?

The renewal of wires has been carried out as per requirements and same is recorded
onboard (renewal maxm 5 years, end for end maxm 2.5 years gap)?
The gangway wires, sheaves etc are frequently greased and maintained?
The stanchions on the gangway or ladder can be locked and are not loose in their
housing?
Is the location of gangway away from working area, cargo area or where suspended
loads may pass above?
Is a lifebuoy equipped with Self Igniting light and buoyant life line available for
immediate use?
Does the top arrangement provide direct access to deck and is this guarded by
handrails and handholds?
Is the gangway of sufficient length to ensure that at maxm. designed operating angle
the lowest platform is not more than 600 mm above waterline on lightest seagoing
condition?
Are the steps of non skid construction and are they free of obstructions or slippery oil
grease etc?
B = THE VISITOR (PILOT, SURVEYOR, CREW ETC)
If the embarking / disembarking person is already short of breath, under influence of
alcohol or drug or afraid to disembark - has this been reported to the Master?
Is the embarking disembarking person with proper PPE and devoid of unnecessary
bags, loose things in the pocket which may hinder his boarding?
C = THE WINCH / MOTOR
Has the winch been tested (along with the gangway) through minimum TWO hoisting
and lowering operations.
Have the lifting arrangements been inspected and is the wire in good condition without
loose strands?
IS the wire is covered with protective sleeve? Has the area been inspected for possible
corrosion?
Is the winch mover, if hydraulic or pneumatic motor, in good order, routinely checked,
inspected and are the connections secure to prevent air hose whipping out.
Is the winch gear box routinely examined for particles in the gear oil and condition of oil
// water ingress?
Were the following closely examined at annual survey - brake mechanism including
condition of brake pads and band brake etc, remote control system, power supply
system, all fittings, davits and pulleys?
Is the gangway wire in conformity with the makers certificate / requirement and
construction criteria? Is the certificate onboard and the wire not more than 5 years
since changed?
D = THE SYSTEMS / CONTROLS
How often are the Gangways and their securing arrangements Inspected by ship staff?
Who carries out the Inspection onboard? (This is in addition to Annual Surveys by
Class etc).
Is this recorded? Where?
The crew rigging gangway are with proper PPE, Life vest and Safety Harness and
supervised by another person on deck at all times.
How often are the Gangways inspected by an independent third party?
Are Gangways inspected whilst in the deployed position?
Who supervises Gangway boarding? Is a ship staff, responsible Officer always
available at the gangway?
Is the lighting sufficient near the embarkation / disembarkation area?
When was the last man overboard drill conducted on the vessel?
Is the maintenance plan for Gangway in place and is it effective?
Is a combination ladder system in use (where the freeboard is more than 9 mtrs)? Is
the angle of gangway too steep?
The Embarkation / disembarkation has been a subject of RISK Assessment and TOOL
BOX meeting both of which have been documented and filed.
