INJURY – FACE S. No. 73 Date 09-Apr-2013 Occurrence 73-1. As part of maintenance of Fire and GS pump, engine room team planned to open the pump on 09-April-13. First engineer was assigned as PIC forming team of total five persons to carry out the planned job. 2. The job started 0900 hrs and pump casing half was removed on 0940 hrs. Pump was inspected together with Chief engineer around 0950 hrs. Wear down of mouth ring was noticed. As result of inspection, it was decided to remove pump shaft assembly for shifting to workshop for maintenance. After inspection, Chief engineer proceeded to E/R workshop for mustering required spare parts of pump shaft assembly for complete overhauling. 3. First engineer asked electrical officer and wiper to prepare trolley crane for lifting pump shaft assembly (Weight – 60 Kgs) around 1035 hrs and continued removing pump shaft assembly. At 1040 hrs pump shaft assembly removed and kept it at side of Fire and GS pump. 4. While electrical officer and wiper preparing trolley crane, all the three persons working on Fire and GS pump were moved away underneath the cover plate being removed on third platform. The height of floor plate on third platform is 5.3m from bottom floor. 5. Wiper operated trolley crane and brought it to location for lowering it down. Electrical officer attempted to shift floor plate all alone (Dimension 1020 mm X 1145 mm ). As soon as he shifted floor plate to one side, he lost control and floor plate fell down. 6. While crane was being prepared for lowering, first engineer was inspecting pump shaft assembly. Due fact that plate dropped down with loud thud, as natural reflexive action, he swung his head sharply and hit his face to fan guard cover of pump Motor and had deep cut on right nostril. Bruise to his neck inflicted from sling attached with motor. Type Impact Primary Accident Personnel Severity Impact Secondary Significant Unspecified Lessons learnt 1. Vessel record of reporting near miss, identifying issues as part of DEVIL hunting regime (Dangerous Event and Irregular look), Stop work activity, safety familiarization of crew members, safety meetings and active participation in company initiated safety campaign were found in order. 2. As part of job planning to work on Fire and GS pump, SMS procedure of discussing the planned job through ‘daily job order’, formal risk assessment and work place safety were followed. Prior commencing the job, team was briefed for job hazards and safety precautions in tool box meeting. 3. All the persons involved in job were donned with adequate PPE. All the five persons were adequately rested and sober while work. The breath test carried out after incident and alcohol content of all persons noted zero. Message to the Industry Removable and unhinged type of floor plates shall be suitably supported to prevent free fall and adequately marked.
