INJURY – FACE S. No. 45 Date 20-Mar-2013 Occurrence 45-MINOR INJURY on FACE - Fitter was cutting a small pipe using portable grinder and was hit by broken pieces of the grinder disc. The safety goggles and Shield Mask were broken and the flying pieces caused laceration about 2 cm. close to his left eyebrow. The fitter was lucky not have a bigger injury. Investigation revealed that the pipe was held by two persons without a rigid support. When the pipe was almost cut, it bent in causing hard abrasion on the disc sides which broke the disc into pieces.. Type Impact Primary Incident Personnel Severity Impact Secondary Significant Property Lessons learnt Pipe cutting should be done preferably using gas torch. If unavoidable then the pipe should be held rigidly on bench vise etc. The cutting equipment must be used in a controlled manner (mechanically held and maneuvered in a control manner). Message to the Industry Accidents happen when routine jobs are done without assessing the Risks and What-If scenarios. Simple jobs usually get taken for granted and are a major cause for misery as no one expects anything to go wrong there.
