Report
A comprehensive report is formulated after all the information is collected on the child from the teacher’s checklist, classroom observation, assessment and parent interview. The report starts with the general information on the child then each area that was evaluated is pointed out with strengths, concerns and suggestions for interventions.The purpose of this report is to communicate results in such a way that the reader will understand the rationale behind the recommendations, and will be able to use the recommendations as practical guidelines for intervention. This report may be presented to the parent, sent to an outside doctor or agency. The report summarizes findings and making recommendations that is best to help the child in the classroom and everyday living.
