Language Access Complaint Process
Language access complaint forms are available on OCEIA ’ s website in English , Spanish , Chinese , Filipino , and Russian .
To request additional languages : Call 415-554-0060 or email language . access @ sfgov . org .
Community members can submit a language access complaint in one of the following ways :

Complete OCEIA ’ s digital form
EMAIL Fill out a complaint form and email it to OCEIA : language . access @ sfgov . org
MAIL
PHONE
Fill out a paper complaint form and mail or drop it off in person at OCEIA ’ s office at 1145 Market Street , Suite # 100 , San Francisco , CA 94103
Call OCEIA ’ s office at 415-554-0600 and share the information by phone
IN PERSON
Visit OCEIA ’ s office at 1145 Market Street , Suite # 100 and work with an OCEIA staff member to submit the complaint in person
