1.3.19 MEDICAL STAFF BYLAWS ” means the Medical Staff Bylaws , the Credentialing & Privileging Manual , and the Rules & Regulations .
1.3.20 “ MEDICAL STAFF LEADER ” means any Medical Staff Officer , Medical Staff Services Department , Department Chair , Section Chief , and committee chair .
1.3.21 “ MEMBER ” means any physician , dentist , oral surgeon , and podiatrist , who has been granted Medical Staff appointment by the Board or any Category I and Category II practitioner who has been granted appointment to the Advanced Practice Provider Staff by the Board .
1.3.22 “ NOTICE ” means written communication by regular U . S . mail , e-mail , facsimile , Hospital mail , or hand delivery .
1.3.23 “ ORAL AND MAXILLOFACIAL SURGEON ” means an individual with a D . D . S . or a D . M . D . degree , who has completed additional training in oral and maxillofacial surgery .
1.3.24 “ ORGANIZED HEALTH CARE ARRANGEMENT ” (“ OHCA ”) means the term used by the HIPAA Privacy Rule which permits the Hospital and Medical Staff to use joint notice of privacy practices information when patients are admitted to the Hospital . Practically speaking , being part of an OHCA allows the members of the Medical Staff to rely upon the Hospital notice of privacy practices and therefore relieves Medical Staff members of their responsibility to provide a separate notice when members consult or otherwise treat Hospital inpatients .
1.3.25 “ PATIENT / SERVICE ENCOUNTERS ” means the exercising of clinical privileges for patients of the hospital ( e . g ., admission , consultation , same-day surgery , and / or primary responsibility for a patient , to include daily rounding with written progress notes , and / or provision of clinical / diagnostic services ). Assignment of the Emergency Department ’ s On-Call shift will also be considered a patient encounter . However , patient referrals for diagnostic or laboratory tests are not considered patient encounters .
1.3.26 “ PERMISSION TO PRACTICE ” means the authorization granted to certain Advanced Practice Providers by the Board or CEO , as applicable , to exercise a scope of practice .
1.3.27 “ PHYSICIAN ” includes both Doctors of Medicine (“ M . D . s ”) and doctors of osteopathy (“ D . O . s ”).
1.3.28 “ PODIATRIST ” means a Doctor of Podiatric Medicine (“ D . P . M .”).
1.3.29 “ PSYCHOLOGIST ” means an individual with a Ph . D ., Psy . D . or Ed . D . in clinical psychology .
