where the individual practiced for at least the previous two years ), ability to exercise the privileges requested , and current professional liability coverage ; compliance with privileges criteria ; and consideration of information from the National Practitioner Data Bank and from a criminal background check and from OIG queries ; c .
d . the applicant demonstrates that ( i ) there are no current or previously successful challenges to his or her licensure or registration , and ( ii ) he / she has not been subject to involuntary termination of Medical Staff membership or involuntary limitation , reduction , denial , or loss of clinical privileges , at another health care facility ;
the applicant shall be subject to any focused professional practice requirements established by the Hospital ;
e . the individual may exercise locum tenens privileges for a maximum of 120 days , consecutive or not , anytime during the 24-month period following the date they are granted . If an individual who has been granted locum tenens privileges has reason to exceed the 120-day maximum time frame , he or she may request a renewal of locum tenens privileges for an additional time period , not to exceed 120 days . Such requests shall be granted by the CEO following review and favorable recommendations by the Medical Staff Chief of Staff , the Chair of the Credentials Committee , and the relevant Department Chair . If any of those individuals has any concerns about the renewal request , the matter shall be forwarded to the full Credentials Committee for review and recommendation ; f .
g . if the individual will require privileges for more than two ( 2 ) 120-day periods , he / she must formally apply for Affiliate category membership ; and
a locum tenens physician / APP whose contract is cancelled does not have rights to a fair hearing .
3.8 Advanced Practice Provider Category 3.8.1 Qualifications a .

The Advanced Practice Provider Category shall consist of Category I and Category II practitioners who provide care , treatment and services to patients at the Hospital . These practitioners , as well as defined Category III practitioners , are subject to the credentialing and privileging process that is outlined in the Credentialing & Privileging Manual .
The Advanced Practice Provider Category is not considered a category of the Medical Staff but is included in the Bylaws for convenient reference only .
3.8.2 Prerogatives Members of this category may : a . exercise clinical privileges granted ;
