ARTICLE 5 PROCEDURE FOR REAPPOINTMENT
5 . A . PROCEDURE FOR REAPPOINTMENT
All terms , conditions , requirements , and procedures relating to initial appointment will , as applicable , apply to continued appointment and clinical privileges and to reappointment .
5 . B . REAPPOINTMENT CRITERIA 5 . B . 1 . Eligibility for Reappointment :
( a ) To be eligible to apply for reappointment and renewal of clinical privileges , an individual must have had sufficient number of patient encounters required by the Medical Staff Category at the Hospital during the previous term of appointment and the required volume of activity required for core / special privileges . An individual must also have :
( 1 ) completed all medical records and be current at the time of reappointment ; ( 2 ) completed all continuing medical education requirements ;
( 3 ) satisfied all responsibilities , including payment of any dues ( if applicable ), fines and assessments ;
( 4 ) continued to meet all qualifications and criteria for appointment and the clinical privileges requested ; and
( 5 ) paid any applicable reappointment processing fee ( if applicable ).
( b ) Members seeking to renew their clinical privileges must have had sufficient patient contacts to enable the assessment of current clinical judgment and competence for the privileges requested . Any member seeking to renew their clinical privileges who has minimal activity at the Hospital must submit such information as may be requested ( such as a copy of their confidential quality profile from their primary hospital , clinical information from their private office practice , or a quality profile from a managed care organization or insurer ) before the application will be considered complete and processed further .
( c ) If a member fails to satisfy eligibility criteria , their appointment and clinical privileges will expire at the end of their current term and they will be ineligible to apply for appointment or clinical privileges for two years , unless an exception is recommended by the MEC and approved by the Board .
