fellows and students and their interface with Medical Staff in the delivery of care, assuring that each participant in a professional graduate education program is supervised in his / her patient care responsibilities by an LIP( s) who has been granted clinical privileges through the Medical Staff process.
2.7 Utilization Management Committee( Hospital committee) a.

Composition: The utilization management committee shall consist of at least two( 2) members of the medical staff. The CMO / VPMA shall appoint the Hospital representatives to the committee.
Responsibilities: This committee shall be responsible for the functions described in section above.
2.8 Quality, Professional Practice, and Peer Review Committee( QPRC) a.

Composition: Membership is described in the Quality, Professional Practice, and Peer Review Policy.
Responsibilities: The committee shall be responsible for the oversight of the activities described in the Quality, Professional Practice, and Peer Review Policy.
2.9 Cancer Committee a.

Composition: Members of the Committee will include representatives from the Active Staff in the following specialties: Surgery, Medical Oncology, Radiation Oncology, Pathology, Radiology, Urology, Gastroenterology, Pulmonology, General Surgery and Gynecology Oncologic Surgery. Additionally, the ACOS Physician Liaison must be a member of this committee. Multidisciplinary staff from the cancer registry, social services, quality services, nursing, pharmacy and administration will also be available to facilitate communication between the Hospital and medical staff.
Responsibilities: The cancer committee will concern itself with the entire spectrum of care for cancer patient admitted to the Hospital.

Information submitted, collected, or prepared by any representative of this or any other healthcare facility or organization or medical staff for the purposes of assessing, reviewing, evaluating, monitoring, or improving the quality and efficiency of healthcare provided;
