Home Health & Community Care
• Home Health : Care for patients in their home includes skilled nursing services such as wound care including wound vacs , blood draws , catheter changes , central line care and IV infusion training and oversight .
• Hospice : Addressing the needs of patients at the end of their life , in their home . We aspire to provide optimum comfort and ensure our patient ’ s caregivers and loved ones are well prepared to care for the patient during this transition .
• Community Care : Provide programs and in-home care for patients to help reduce their chance of admission or readmission to the hospital .
