( d) A decision by the Board to grant appointment will be for a period not to exceed three years.
( e) Any final decision by the Board to grant, deny, modify, or revoke appointment or clinical privileges will be disseminated to appropriate individuals, to include the applicant, stating the reason for the decision( if unfavorable to the applicant) and, as required, reported to appropriate entities. This notification will take place no later than thirty( 30) days after the Credentialing and Privileging Committee’ s decision.
( f) Upon approval by the Board and completion of Medical Staff orientation, a new provider may begin engaging in direct patient care. Prior to the Board’ s decision and notice of an effective date, the applicant is not allowed to provide direct patient care.
3. B. 8. Time Periods for Processing:
Once an application is deemed complete, the completed credentialing file is expected to be processed and voted on by the Credentialing and Privileging Committee within one hundred and twenty( 120) days of its submission. If the file is not completed within one hundred and twenty( 120) days, the application will need to be resigned, redated and verifications performed again to bring the file into compliance prior to the Credentialing and Privileging Committee.
