Committee Chair will review the application to determine that the applicant fulfills the threshold criteria for appointment and privileges and will also determine whether the application is forwarded as a Category 1( Streamlined) or may change the designation to Category 2( Traditional).
( b) If the Credentials Committee Chair determines to keep the designation as Category 1( Streamlined), the Chair may choose to act on behalf of the Credentials Committee, make a decision to recommend appointment, and then forward the the application to the MEC for review and recommendation. If the designation is Category 2( Traditional), the Credentials Committee will review and act on the application at its next regularly scheduled meeting, unless a special meeting is convened.
( c) The Credentials Committee may rely on the expertise of the department chair / section chief, any member of the department / section, or an outside consultant, if additional information is required regarding the applicant’ s qualifications.
( d) After determining that an applicant is otherwise qualified for appointment and privileges, if there is any question about the applicant’ s ability to perform the privileges requested and the responsibilities of appointment, the Credentials Committee may require the applicant to provide information regarding their health status and / or require a fitness for practice evaluation by a physician( s) satisfactory to the Credentials Committee. The cost of the health assessment shall be borne by the applicant. The results of this evaluation will be made available to the Committee. Failure of an applicant to provide any of the requested information or to undergo a fitness for practice evaluation within a reasonable time after being requested to do so in writing by the Credentials Committee will be considered a voluntary withdrawal of the application and all processing of the application will stop.
( e) The Credentials Committee may recommend the imposition of specific conditions related to behavior, health or clinical issues. The Credentials Committee may also recommend that appointment be granted for less than the full appointment term in order to permit closer monitoring of the applicant’ s compliance with any conditions.
3. B. 6. Medical Executive Committee Recommendation:
( a) If the application is designated Category 1( Streamlined) it is presented to the MEC, which may meet in accordance with quorum requirements established for expedited credentialing. The Chief of Staff has the opportunity to determine whether the application is forwarded as a Category 1( Streamlined), or may change the designation to a Category 2( Traditional). If the application receives a recommendation from the MEC that is adverse or has limitations, the application is ineligible to continue in the streamlined process and must follow the process for traditional applications.
