( e) The Board’ s determination regarding whether to grant a waiver is final and no appeal rights are afforded to the applicant.
( f) If a waiver is granted that does not specifically include a time limitation, the waiver is considered to be permanent, and the individual does not have to request a waiver at subsequent reappointment cycles.
( g) A determination to grant a waiver does not mean that appointment will be granted, only that processing of the application may continue
( h) No applicant is entitled to a waiver or to a hearing if the Board determines not to grant a waiver. A determination not to grant a waiver of eligibility requirements is not a“ denial” of appointment or clinical privileges and the individual who requested the waiver is not entitled to a hearing. Rather, the individual is ineligible, either in part or in whole, to request appointment or clinical privileges. A determination of ineligibility is not a matter that is reportable to either the state licensure board or the National Practitioner Data Bank. A determination to grant a waiver in a particular case is not intended to set a precedent for any other applicant or group of applicants.
( i) Any applicant whose application is not processed due to failure to meet eligibility criteria may not reapply for application for one year, unless an exception is granted by the Credentials Committee. Only after the Credentials Committee reviews and grants the waiver for reapplying will Medical Staff Services send the applicant the application.
2. A. 3. Factors for Evaluation:
The six ACGME general competencies will be evaluated in the initial grant or renewal of clinical privileges as well as serve as the basis for peer references, as reflected in the following factors:
( a) relevant training, experience, and demonstrated current competence, including medical / clinical knowledge, technical and clinical skills, clinical judgment and an understanding of the contexts and systems within which care is provided;
( b) adherence to the ethics of the profession, continuous professional development, an understanding of and sensitivity to diversity, and responsible attitude toward patients and the profession consistent with the applicant’ s professional society and Renown Health’ s policies;
( c) good reputation and character;( d) ability to safely and competently perform the clinical privileges requested;
( e) ability to work harmoniously with others, including, but not limited to, interpersonal and communication skills sufficient to enable them to maintain
