( v) agree to fulfill all responsibilities regarding emergency call for their specialty; and
( vi) document compliance with immunization and health screening requirements( e. g., TB testing, mandatory vaccines, and infectious agent exposures), as may be updated from time to time;
( 11) have not been separated from employment with Renown Health, or any affiliated entity, such that they are ineligible for re-employment or re-hire; and
( 12) if seeking to practice as an advanced practice provider, meet any state and hospital requirements for a written agreement.
( b) In order to be eligible for continued appointment and privileges, individuals must demonstrate satisfaction of the above threshold eligibility criteria, as applicable, on an ongoing basis.
2. A. 2. Process for Waiver of Threshold Eligibility Criteria:
( a) An applicant who does not satisfy one or more of the threshold eligibility criteria will be notified and may request a waiver as outlined below. The applicant requesting the waiver bears the burden of demonstrating that they are otherwise qualified and exceptional circumstances exist( e. g., there is a need at Renown Health for the services provided by the applicant). Waivers of threshold eligibility criteria will not be granted routinely.
( b) A request for a waiver will be submitted to the Credentials Committee for consideration. In reviewing the request for a waiver, the Credentials Committee may consider the specific qualifications of the applicant in question, input from the relevant department chair, and the best interests of the Hospital and the communities it serves. Additionally, the Credentials Committee may, in its discretion, consider the application form and other information supplied by the applicant. If necessary, the Credentials Committee can request an interview with the applicant for itself or a designee to obtain any further information in order to reach its decision.
( c) The Credentials Committee will forward its recommendation, including its reasons, to the MEC. Any recommendation to grant or deny a waiver must include specific reasons for the recommendation.
( d) The MEC will review the recommendation of the Credentials Committee and make a recommendation to the Board regarding whether to grant or deny the request for a waiver. Any recommendation to grant or deny a waiver must include specific reasons for the recommendation.
