INTRODUCTION

The CurePSP Prepare to Care workbook is an interactive log of personal and medical history, important contact information and care considerations specific to a diagnosis of progressive supranuclear palsy( PSP), corticobasal degeneration( CBD) or multiple system atrophy( MSA).
The Prepare to Care workbook is meant to assist people with PSP, CBD or MSA and their families to keep important and relevant information in one dedicated and organized place.
How to use this workbook:
CurePSP ' s mission is to raise awareness, build community, improve care and find a cure for PSP, CBD and MSA.
We are here to support you through the journey with PSP, CBD or MSA and offer brain donation assistance, virtual and regional support groups, the Quality of Life Respite Grant, family conferences, webinars and other resources. • You can bring this with you to medical appointments, visits to the emergency department, planned or unplanned hospitalizations or stays in a rehabilitation / nursing facility.
• Have anyone who provides care( e. g., family members, professional in-home caregivers, nursing staff) refer to it to learn about the person’ s diagnosis and care needs.
• Utilize the pocket of this workbook to hold the CurePSP Grab & Go( available at www. psp. org / ineedsupport / resources), brain donation paperwork( learn more at www. psp. org / braindonation-program), advance care directives and any other important documents related to care.
• Update the information contained in this workbook as needed.
