Please let us know if you have ideas or suggestions to improve our programming.” A message from Ash Carrel in Social Services. On an annual basis we provide residents, and responsible parties, with an updated copy of resident rights, rules, and regulations for the facility. If you would like additional copies, please notify Social Services and one can be mailed or e-mailed to you. You may also pick up a copy from the Social Services office. According to our federal regulations, all residents have the right to formulate an Advanced Health Care Directive (AHCD). An AHCD allows individuals to make their health care wish- es known (such as which type of medical treatment they would refuse or accept). This di- rective requires notarization, but does not need to be done by an attorney. Anyone completing an AHCD must be able to understand the document’s purpose. This document is completed in advance of any need, so loved ones and physicians know your wishes in the event you are not able to make your wishes known. If you have questions or would like to complete one for yourself, please call Ash Carrel at Parkview Health Care Facility, 417-326-3000.
