Community Outreach Project : Dallas At-Risk Neighborhoods
Patients with chronic conditions living in the primary service area for the Dallas hospital accounted for a significant number of Baylor University Medical Center Emergency Department ( ED ) registrations for patients seeking medical care . These patients ’ health issues could have been addressed by primary care providers ( PCPs ). An analysis of this cohort of patients living the underserved and at-risk Dallas ZIP codes of 75215 , 75216 and 75217 , enabled hospital leadership to identify two primary areas of opportunity : hardwire a process for the patient to see a community health worker from Baylor Scott & White Health ’ s Primary Care Connection ( PCC ) before being discharged from the ED in order to arrange a follow-up appointment with a PCP for the patient ; and reduce the number of repeat episodes of care .
First Responders / EMS Outreach
QUARTERLY STEMI CASE REVIEW
Interventional cardiologists on the medical staff of Baylor Scott & White Heart and Vascular Hospital – Dallas met with teams of first responders / EMS each quarter to review at least three unique STEMI cases from the previous quarter . Each STEMI case review included an extensive feedback component comprised of culprit issue , procedure performed and discharge disposition . Survivors of these cases were also invited to attend with leadership from the corresponding EMS team to tour the catheterization lab .
Other educational efforts in Dallas and Fort Worth included inviting EMS to stay with their patients in the cath lab and watch the outcome of their pre-hospital efforts .
During fiscal year 2018 , a hospital team tracked 100 percent of ED patients meeting specific criteria received a PCC order before being discharged . Data showed an increase in patient compliance for kept appointments from a baseline of 68 percent to a high of 95 percent . After reviewing these results , additional interventions continue to be implemented .

“ LIVING WITH ...” COMMUNITY EDUCATION SERIES
“ Living with ...” Community Education Series continued in fiscal year 2018 . Several Living with AFib and Living with Heart Failure evening events presented by physicians were held in Dallas and Fort Worth . Additional speaking topics were presented throughout the Dallas and Tarrant County areas at various organizations including churches and organizations .

National EMS Week luncheon and cath lab demonstrations
NATIONAL EMS WEEK
During National EMS Week in May , Baylor Scott & White Heart and Vascular Hospital – Dallas ’ cardiology staff teamed up with Baylor University Medical Center staff to host lunch and demonstrations of cath lab processes when a patient experiences a heart attack . Additionally , an inservice was arranged for Tarrant County area EMS during the week . During the demonstrations , EMS personnel from across North Texas met members of the cath lab team , discussed what happens when they transmit a patient ’ s EKG and learned how pre-hospital activation helps to improve health outcomes for the patient . Over the course of four events , the hospital clinical teams were able to connect with nearly 250 EMS providers and thank them for their service to the community .
