General Registration Information
How can I register for programs ?
Online : Register online through WebTrac at register . nssra . org .
Email : Email your registration form to registration @ nssra . org . The registration form can be found on on pages 57 - 58 of this guide , or online at www . nssra . org / register .
Fax : Fax your registration form to ( 847 ) 509 – 1177 .
Drop Off : Drop off your registration form to NSSRA , 3105 MacArthur Blvd ., Northbrook , IL 60062 . The NSSRA Office is open Monday - Friday , 9:00 am - 6:00 pm .
Mail : Mail your registration form to NSSRA at 3105 MacArthur Blvd ., Northbrook , IL 60062 .
To register for several programs , please put the additional program information on the spaces provided on the back of the registration form . Please complete the Fee Information section for all programs on the registration form in the space provided .
Length of Programs : Most weekly programs run for eight weeks , however , programs with this after the regular program dates . symbol meet prior to and / or
Min / Max Information : Program min / max numbers reflect the minimum number of participants required to run a program and the maximum number of participants allowed to register for a program . Some programs may be offered in more than one section of the program guide according to the designated age group .
AIF / SIF Forms : Participants are required to complete an Annual Information Form ( AIF ), found on our website at www . nssra . org / forms . The AIF must be completed and filed with the NSSRA office with the completed registration form in order for a participant to join any NSSRA program or event . The AIF contains important participant information which is necessary for NSSRA staff to plan and execute safe and enjoyable programs . Please also complete the Seizure Information Form ( SIF ), if applicable . The SIF can also be found online at www . nssra . org / forms .
Payment must be included . You may enclose a check payable to NSSRA or charge the total amount on Mastercard , Visa , Discover or American Express . Please include card type , card number , expiration date , CVV # ( on back of credit card ) and your signature . Any outstanding balance must be paid before registration for next season can be accepted .
Register early . NSSRA program registrations are processed on a first come , first served basis . We understand that it is not always possible to meet a deadline . However , if we receive your registration after our deadline , your registration will be held until we can determine if accommodations for the participant can be made . These accommodations include transportation capacity , staff assignments and space limits at particular events . Please be assured that we will do our best to fit you in or find another suitable program .
The best way to ensure participation in a program ( s ) is to register as early as possible . The registration deadline is Friday , February 24 .
