avec le CLSC . Tout évolue très vite en médecine interne , car les patients y séjournent peu longtemps . Tania doit travailler rapidement pour déterminer les besoins des patients et prévoir des services dans la communauté .
Il faut faire beaucoup de téléphones pour un seul transfert . Certaines situations sont complexes . Parfois , il faut faire preuve d ’ imagination pour trouver des solutions .
PARLER À LA FAMILLE 11 h 20 . Tania rencontre une famille . Une
dame âgée pourra rentrer chez elle avec l ’ aide du CLSC local , mais les services ne seront disponibles que dans quatre jours . Tania doit s ’ assurer que le fils de la patiente peut compenser l ’ absence de services communautaires pendant cette période .
Les personnes âgées vivent un traumatisme émotionnel lorsqu ’ elles sont retirées de chez elles . Nous tentons donc le plus possible de maintenir les patients à domicile . Nous cherchons à promouvoir leur autonomie et à les aider à maintenir leur dignité .
PAUSE-REPAS 11 h 30 . Tania dîne avec ses collègues Diana D ’ Addio et Sarah Smith ,
dans la salle de réunion de l ’ unité des services sociaux , au rez-dechaussée du site Glen .
J ’ essaie de dîner entre 11 h 30 et 12 h 30 . J ’ ai besoin de cette période pour refaire le plein d ’ énergie et placoter avec mes collègues . Nous rions et nous relaxons .
PETITE MARCHE 13 h 15 . Tania s ’ en va rencontrer une femme de 44 ans qui souffre
de sevrage alcoolique . Elle lui demandera si elle veut se prévaloir de ressources de désintoxication . D ’ autres patients peuvent avoir besoin d ’ information sur les ressources financières ou les refuges pour sans-abri . g

discussing a case with the CLSC . Internal medicine is a fast paced environment , where patients usually stay for a short period of time . Tania has to work quickly to determine patients ’ needs and to implement services in the community .
There are a lot of telephone calls involved for just one relocation . Some situations are complex . Sometimes you have to think outside the box to find solutions .
TALKING TO FAMILY 11:20 a . m . – Tania has a family meeting . An elderly lady will be able return home with help from the local CLSC , but services will only be available in four days . Tania has to make sure that the patient ’ s son can compensate for the lack of community services during that period .
Removing an elderly person from their home is emotionally traumatic , so we try as much as possible to keep patients at home . Our goal is to promote their autonomy and to help them maintain their dignity .
LUNCH BREAK 11:30 a . m . – Tania sits down for lunch with colleagues Diana
D ’ Addio and Sarah Smith in the meeting room of the Social Services unit on the Glen ’ s main floor .
I do try to have a lunch break between 11:30 and 12:30 . I need this time to refuel and chatter with my colleagues . We laugh and relax .
WALKING 1:15 p . m . – Tania is on her way to see a 44-year-old woman
suffering from alcohol withdrawal . She will ask if the patient is interested in detox resources . Other patients may need information about financial resources or homeless shelters .
A large part of my job is to educate patients and family members about what services and resources are available and how to access them . I ’ m the link between the hospital and the community . gg
