• To enroll in EZ PAY , simply add ( or update ) your credit card information on file – this can be done in person or over the phone .
• Your current monthly balance will be charged to the card on file on the 20th of each month .
MEMBER BILLING : Billing Inquiries , Statement Disputes , Medical Leave Requests and Cancellations must be submitted in writing to Billing @ Eaglebrooke . com .
