CHAPTER II Appendix 3b : Student’s behavior in class and school – form (twice a year) STUDENT’S BEHAVIOUR IN CLASS AND SCHOOL FORM Assess the frequency of the below behaviours. Use the scale from 1 to 6 where 6 means- “always” and 1 means- “never”.
