Gather Up Your FIVE( 5) Documents to be Uploaded to Your Online Application
Document # 1: Birth Certificate or Valid Passport
Document # 2: Immunization Record with student ' s name on record OR Religious Exemption
Document # 3: Deed( if you no longer carry a mortgage on your home), Full Current Lease with Signature Page that is not expired, OR Full Current Mortgage Statement
Document # 4: Full Current Utility Statement, i. e., gas, water or electricity
Document # 5: Current / Valid PA Driver ' s License AND PA Department of Transportation Change Card if you have not yet updated it to your new home address.
