Why This Information Is Needed
Millcreek Township is launching a new initiative to strengthen communication with Homeowner Associations( HOAs) throughout the community by collecting updated HOA contact information. Over the past several years, the Township has received frequent requests from residents seeking HOA contact details.
Who Should Complete the Form
While the Township maintains partial records, many associations have experienced leadership changes, making it difficult to ensure information remains current. To address this, the Township has created a public GIS-based HOA Representative Form that allows associations to easily submit or update their information.
Accurate HOA contact records play an important role in Township operations. Updated information helps the Township share timely planning, zoning, and development updates; provide neighborhoodspecific notifications; coordinate outreach during construction or infrastructure projects; and maintain reliable internal records. illcreek

HOA representatives are encouraged to complete the form if they have not previously submitted their contact information, if their association has experienced a change in leadership or board members, or if their neighborhood has recently formed a new HOA.
All information collected is used for Township communication purposes only and is not shared publicly.
Where the Form Will Be Available
Once finalized, the form will be available on the Millcreek Township website under Planning and Development and shared to help spread awareness. HOA representatives may submit their information online through the QR code to the left.
