Reliability and validity of clinical assessments and radiological imaging techniques for the diagnosis of Patellofemoral Pain Syndrome : A Systematic Review
Currently there is no gold standard technique to diagnose patellofemoral pain syndrome ( PFPS ). The purpose of this review was to investigate the diagnostic accuracy and validity of the current clinical assessment tools and radiological imaging techniques to diagnose PFPS in patients presenting with pain in the patella area . The database search revealed 25,433 studies , of which 5 were eligible for this review . Included in these studies were 540 participants [ 234 PFPS , 306 controls ]. All 5 studies scored low risk of bias on the QUADAS quality scale . From the 5 studies , 14 singular diagnostic tests and 9 combination diagnostic tests were assessed for diagnostic accuracy measures . For single tests the highest sensitivity [ 90.38 %, CI 95 %] was that of the squatting test , and highest specificity [ 88.46 %, CI 95 %] was patella compression test . For combination tests , history and physical examination scored highest sensitivity [ 92 %, CI 95 %] and specificity [ 92.65 %, CI 95 %]. It was concluded that combination tests of history elements and physical examination proved to have good diagnostic tendencies for the diagnosis or exclusion of PFPS for specific cohorts . Despite these findings , until a consensus is agreed regarding a reference standard for PFPS , diagnostic accuracy studies will suffer from bias .
