Treatment plan
The patient was explained about the present
situation of porcelain fracture and multiple
carious teeth. Also biomechanical aspect of the
prosthesis was discussed with the patient for a
long term treatment success. The treatment
options like fixed partial denture were not
feasible biomechanically unless supported by
implants. But the financial constraints let the
patient decline the implant supported
prosthesis. The removable cast partial denture
was then agreed upon by the patient and the
following treatment plan was thus formulated
and executed.
Removal of the earlier fixed prosthesis was
done (Image 2). Oral prophylaxis was carried
out, restorative procedures of the carious teeth
was performed, followed by endodontic
treatment of 18, 13, 21, 22 and 26. New
diagnostic casts were made and surveyed for
planning cast partial denture. The teeth
required for engaging cast partial denture
components were surveyed for undercut
locations and depth with the help of dental
surveyor. Full veneer crowns restoring the
compromised abutment teeth and correcting
crown contours and occlusal plane were
decided. Shade selection, crown preparation
for 18, 13, 21, 22, 26, 28 were done. Provisional
restorations were given (Image 5).

Wax pattern of crowns were surveyed for
fulfilling requirement of path of insertion of
planned prosthesis. Try-in of posterior metal
and anterior metal-ceramic crowns was done
(image 6, 7) followed by cementation after
minor adjustments and polishing.
