Health Risk Assessment
Health Risk Assessments should be kept confidential and used to help individual club members learn
which health needs they can address in their own lives. If people are willing to share certain topics that
they struggle with, the results of this assessment can be used to guide the focus of regular club meetings
and club-sponsored events.
