Citizens Memorial Health Care
Foundation’s (CMH) Health Transit
program provides non-emergency
health transportation on a onetime or long-term basis to help you
remain at home and independent.
Health Transit operates Monday
through Saturday from 6 a.m. to
4:30 p.m. Our caring drivers will
pick you up at your home, take you
to your health-related destination
and return you to your home.
The staff at Health Transit
take pride in everything they
do. They will contact you in
advance to work out all of the
logistics of the trip, such as time
and duration of the appointments.
Their professionalism, in conjunction with top-of-the line
fleet vehicles, will make certain
that you arrive to your
appointments in a safe and
timely manner.
CMH Health Transit services
are available to residents in Cedar,
Dade, Dallas, Hickory, northern
Greene, Polk and St. Clair
Counties.
Health Transit accepts private
paying patients and those qualifying
for Medicaid. For Medicaid
transportation trips, contact
Logisticare at 1-866-269-5942. For

Rick is the supervisor and coordinator for CMH Health Transit. He
is responsible for all scheduling
and making certain that all aspects
of a transport runs smoothly. He is
also responsible for working with
Logisticare, Medicaid and billing.
He also ensures that passenger
transports are handled in a safe and
timely manner.
Perry is responsible for ensuring
that everyone who uses Health
Transit’s services will get to his/
her destination in a safe and timely
manner. Perry works with his passengers to make sure all of the logistics of each trip are working as
smoothly as possible.
Ronnie is responsible for making
certain that all the passengers who
utilize CMH Health Transit make it
to their destination, whether it is a
doctor, appointment, long-term
care facility or home, in a safe and
timely manner. He helps with all
non-emergency transports and hospital discharges.
