REACHING THE MOST VULNERABLE: HEART SUPPORTS SENIORS EXITING HOMELESSNESS

Homelessness disproportionately affects vulnerable populations, including older adults with complex medical needs. In Arlington, the HEART program plays a critical role in reaching and supporting these individuals through targeted outreach and housing services.
In one case, a woman in her late 50s was living out of her car while undergoing medical procedures for her eyes. Without a safe place to recover or store medications, her health was rapidly deteriorating. HEART— comprising Arlington Police Department officers, crisis intervention specialists, and DRC outreach staff— enrolled her in services and expedited her housing match. Just in time for Christmas 2024, she moved into a safe, stable home where she could focus on healing.
In another instance, HEART encountered a man in his 70s who was unsheltered and managing ongoing heart problems. After enrolling him in services, HEART helped him collect essential documents to qualify for housing. He has since been matched with a housing program and is now working with a case manager to secure permanent housing.
These stories highlight the vital role HEART plays in preventing medical crises and restoring dignity for Arlington’ s most at-risk residents. With continued support, HEART ensures that no one is left behind— especially those who need help the most.
