Early Registration Fee: $20 PER PERSON (no refunds)
Early Registration Deadline: FRIDAY, May 19, 2017
Fourth District PTA must receive your registration form and payment on or before this date to be eligible for
the early registration fee. Please allow time, if mailing.
Note: Your fee includes a continental breakfast, lunch, access to our vendor fair and admission to morning and
afternoon workshops. Electronic handouts will be available. Hard copy handouts provided upon request only if
you register before Friday, May 19, 2017.
All Late Registrations or at the door payments: $25 per person
Walk-in registrations will be accepted only if there is space.
Vendor Fair Access ONLY: $10 per person
Access to visit our vendor fair and lunch only NO WORKSHOPS NO ACCESS TO ELECTRONIC

REGISTRATION PROCEDURES
1. Online at www.fourthdistrictpta.org. Payment must be made with a credit
card. Payments may be made via PayPal.
2. Print and mail or deliver the completed form with your payment to:

Your registration and payment must be in Fourth District Office by 2:00PM on Friday, May 19, to be eligible for the early
