DISTRICT 6: Counties of Frontenac, Haliburton, Hastings, Leeds and Grenville, Lennox and
Addington, Northumberland, Peterborough, Prince Edward and Victoria:
1 position
DISTRICT 7: Counties of Dundas, Glengarry, Lanark, Prescott, Renfrew, Russell and
Stornmont, and the Regional Municipality of Ottawa-Carleton: 2 positions
DISTRICT 8: Territorial districts of Algoma, Cochrane, Manitoulin, Nipissing, Parry Sound,
Sudbury, and Timiskaming: 1 position
DISTRICT 9: Territorial districts of Kenora, Rainy River and Thunder Bay: 1 position
A
n election will be held on Tuesday, Oc-
tober 9, 2018 to elect Council represen-
tatives to the College of Physicians and
Surgeons of Ontario for Districts 6, 7, 8
and 9.
Members interested in becoming candidates must
submit their nomination and conflict of interest forms
to the Registrar no later than 4:00 p.m. Tuesday, Au-
gust 21, 2018.
The nomination form must bear the signature and
registration numbers of at least five members eligible
to vote in the electoral district in which the candidate
is nominated.* The candidate nomination and conflict
of interest forms are included in this issue. Additional
forms can be obtained by calling Vanessa Clarke at
(416) 967-2600 ext. 773 or 1-800-268-7096 ext. 773,
or by downloading them from the College website at
www.cpso.on.ca.
On-line voting will be open from Tuesday, September
18, 2018 to Tuesday, October 9, 2018 at 4:00pm. Any
votes/ballots received later than 4:00 p.m. on October
9, 2018, will be considered invalid. Further information
about the voting process can be found at www.cpso.
on.ca.
*We recommend that seven nominators be provided, in the
event that a nominator does not meet the criteria set out in
the nomination form.
