Notice of Election in Districts 5 and 10 on Tuesday, October 10
District 5: County of Simcoe; the District Municipality of Muskoka and the
regional municipalities of Durham, Peel and York: 2 positions
District 10: City of Toronto: 4 positions
A
n election will be held on Tuesday, October 10, 2017 for the purpose of electing Council representa-
tives to the College of Physicians and Surgeons of Ontario for Districts 5 and 10.
Members interested in becoming candidates must submit their nomination and conflict of interest
forms to the Registrar no later than 4:00 p.m. Tuesday, August 22, 2017.
The nomination form must bear the signature and registration numbers of at least five members eligible to vote in
the electoral district for which the candidate is nominated.* The candidate nomination and conflict of interest forms
are included in this issue. Additional forms can be obtained by calling Tanya Terzis at (416) 967-2600 ext. 545 or
1-800-268-7096 ext. 545, or by downloading them from the College website at www.cpso.on.ca.
Online voting will be open from Tuesday, September 19, 2017 to 4:00 p.m. Tuesday, October 10, 2017. Any
votes/ballots received later than 4:00 p.m. on October 10, 2017, will be considered invalid. Further information
about the voting process can be found at www.cpso.on.ca.
