Time to renew your certificate of registration
Don’ t be late!
On April 17 2017, the online feature for completing your annual renewal will be available. As always, it is critical that you provide us with your most up-to-date contact information, including your email address, fax and phone numbers. Before April 17, please login to the secure section of our website( Member / Applicant Login) and make sure that your information is up to date.
Please note: When you login, you will be asked to change your password and answer new security questions. This password reset and security question update helps us ensure the best protection of your information. You can view an electronic version of your renewal notification by clicking on View My Documents within our secure website. Once you have completed your renewal, and your fee payment has been fully processed, you will receive an email that your fee receipt as well as your College membership card are available from our secure website for you to print.
Worried about making an online payment? You do not have to submit your payment online; the option to pay by cheque is still available. Just complete and return the invoice page of your notification along with your payment. Allow adequate time for delivery to make sure your payment is received by the College before the deadline.
New this year This year, you will be able to view the renewal feature and see at a glance whether you have completed your renewal. If you haven’ t completed it, it will show whether the survey, payment or both are still outstanding and there will be direct links to enable immediate completion of the outstanding items.
Important Information If you are not receiving email reminders, you may have unsubscribed. Make sure you review the information on page 22 to ensure you are getting important information from the College. By early May, if you haven’ t completed your renewal online, you will receive your renewal notification by mail, with instructions for completing your renewal. Contact Physician Advisory Service at 416-967-2603 or 1-800-268-7096 ext. 603 if you need this information and are having problems viewing it online. The deadline for completing your annual renewal, which includes submitting both the survey and payment, is 5 pm EDT on Thursday, June 1, 2017. To avoid any late fees, do not miss this important date.
